District Health Information

Health Regulations

In accordance with State regulations, South Brunswick School District requires pupils new to the District, students entering Kindergarten, students trying out for athletic teams, students participating in Color Guard, marching band or dance and those applying for working papers to undergo a physical examination. Physical examinations are strongly recommended for students in grades 5 and 8. Examinations must be completed by a private physician. In the absence of a private physician, parents are encouraged to speak with the school nurse.

The school nurse can administer medication that is packaged in the original container and is accompanied by the District's Medication Form, which must be completed and signed by the child's doctor and parent or guardian. Forms can be obtained through your school nurse or by downloading the information that appears below.

Additionally, each year the school nurse conducts the following screenings:

Height and Weight - All Grades
Blood Pressure - Grades K, 3, 5, 6-12
Vision - Grades K, 2, 4, 6, 8 and 10
Hearing - Grades K-4, 6, 8 and 10
Scoliosis - Grades 5, 7, 9 and 11
Mantoux screening as per current State guidelines.

Immunizations

Immunizations are required for entry or attendance in school to comply with Chapter 14 of the New Jersey Sanitary Code.

In 2008, the State announced various changes to its immunization code.

The following immunizations are now required for children attending child-care, Pre-school or Pre-Kindergarten programs beginning September 2008:

  • Children ages 2-11 months: 2 doses of Pneumococcal conjugate vaccine (PCV) administered between the ages of 2-11 months of age
  • Children ages 12-59 months: 1 dose of Pneumococcal conjugate vaccine (PCV) administered after the first birthday
  • Children ages 6-59 months: 1 dose of Influenza vaccine annually between September 1- December 31

Additionally, students entering or attending Grade 6 who were born on or after January 1, 1997 must receive two additional immunizations:

  • Diphtheria, Tetanus Toxoids and Pertussis Vaccine (Tdap): 1 dose after turning 10 years old and given no sooner than 5 years since the last DTP/Dtap or Td vaccination. The doctor may elect to defer this vaccine until the child turns 11 years old.
  • Meningococcal Vaccine: 1 dose after turning 11 years old.

Students transferring into a New Jersey School from out of state or out of the country who were born on or after January 1, 1997 must also receive the above additional two vaccines regardless of grade entered.

Questions regarding the changes in the requirements should be discussed with your private physician.

Please check with your physician or school nurse to ensure that your child is adequately immunized for their grade. Failure to provide a complete immunization record can result in the child’s exclusion from school. The school nurse reviews all immunization records prior to a student’s admission. Parents or guardians are encouraged to contact the school health office to speak about an individual child’s immunization needs.

Immunization waivers are honored for health or religious reasons when proper written documentation is shared with the school nurse.

You may download the applicable Health History form that seeks information about your child's medical history, complete it and present the document at the time of registration.
